For older people, often the first number (systolic) is 130 or higher, but the second number (diastolic) is less than 80. This problem is called isolated systolic hypertension, which is due to age-related stiffening of the major arteries. It is the most common form of high blood pressure in older people and can lead to serious health problems (stroke, heart disease, eye problems, and kidney failure) in addition to shortness of breath during light physical activity, lightheadedness upon standing too fast, and falls. Isolated systolic hypertension is treated in the same way as regular high blood pressure (130 or higher for the first number, or 80 or higher for the second number) but may require more than one type of blood pressure medication. If your doctor determines that your systolic pressure is above a normal level for your age, ask how you can lower it.
We asked clinicians from Beth Israel Deaconess Medical Center’s CardioVascular Institute how they advise their patients to keep blood pressure under control. While medication is the right solution for some people, the good news is that lifestyle changes can help reduce — and in some cases replace — the amount of medication needed. It’s a good place to start.
What these scientists found was that while drug treatments for mild hypertension may be effective at lowering blood pressure, they were not effective in reducing overall mortality. Put more bluntly, hypertension patients in these studies died at about the same rate whether they took medication or not. These findings reaffirm an important health principle: Treating the symptoms of disease is not the same thing as causing health.
Editor’s Note: Considerable controversy exists about whether fat or cholesterol are, per se, drivers of atherosclerosis. They are implicated in some studies, while others indicate that quality of fat, and placement in a wider dietary pattern, may be more significant to ultimate impact. What seems clear, however, is that a diet high in animal products, sugar, and processed foods is often a recipe for high blood pressure and heart disease.
It’s a common question among our guests at the Pritikin Longevity Center, who are taught how devastating the high-salt U.S. diet is to our blood pressure and overall health. Searching for alternatives, people often ask: What about salt substitutes with potassium? And what about MSG? Isn’t it a bad choice? Here are answers, some of which may surprise you.
Knowing what changes to make is only the first step; following through in sustainable, practical ways is the tougher aspect of healthy living. This is where health coaching, one of the fastest-growing professional fields, can make all the difference. Many patients know what diet and lifestyle changes are needed, but they have trouble following through day to day. A health coach can provide the more frequent support and accountability that a general healthcare provider cannot.
Be Mindful. Studies show that a regular meditation practice can help manage hypertension. So create your own practice: Mindfully brush your hair or rub lotion on your arms and legs. Or simply repeat a mantra with your eyes closed and your body in a relaxed posture. Being mindful means focusing on all of the sensations of the present moment (touch, smell, and sound), such as movement of a brush through your hair.
If you cannot be sure that your diet includes sufficient potassium, consider supplementation. However, including potassium in your diet gives better results, as an intake from food can be accessed by the body more efficiently. Potassium-rich foods include squash, sardines, salmon, raisins, potatoes, organs, pears, legumes, beets, bananas, carrots, and apricots, among many other fruits and vegetables.
Exercise. Some patients will cringe at the suggestion of exercise, because they envision a chronic cardio scenario like a mouse on a running wheel. Spread the good news: exercise of all kinds—endurance, dynamic resistance, HIIT, isometric resistance—has the potential to reduce blood pressure (38). Whatever exercise your patients will actually do on a regular basis is the best recipe for success. In patients with extreme hypertension, be cautious with exercises that may further increase blood pressure to an unsafe zone (39).
To get rid of high blood pressure apples are very beneficial. Apples are rich in zinc which prevents hypertension and also control the level of high blood pressure. Magnesium supplements were found to modestly lower blood pressure—by about 2 points in both systolic and diastolic pressure—in a 2016 analysis of 34 clinical trials. A 2017 analysis of 11 clinical trials reached a similar conclusion. It looked at the effect of magnesium supplements (365 to 450 milligrams a day) on blood pressure in people with diabetes, prediabetes, or insulin resistance. Very curious to see if Chris has any thoughts on this; I have had similar experiences with over 400 mg of magnesium in any form.. I’ve tried various chelates versions. I’m also wary of mega-dosing with supplements. My father has tried all kinds of mega-doses of all these things over the last decade to get off meds and still ends up with scarily high bp after a month or so off meds. Everything in moderation and I think sometimes that has to include conventional medication. Side effects at this late stage in his life can’t be as bad as a potentially debilitating stroke 🙁
Get some sleep. Less sleep has been linked to increased risk of hypertensions and general cardiovascular problems. Even a 45-minute nap during the day has been shown to lower blood pressure. A persons blood pressure naturally rises and falls during the day, the lowest point usually being during the middle of the night and the highest in the middle of the afternoon.

There are many popular medical myths about high blood pressure. For example, many physicians believe that high blood pressure is an “inevitable consequence of aging;” that the “only viable treatment option for high blood pressure patients is medication”; that high blood pressure patients must take their medications “for the rest of their lives”; and, worst of all, that high blood pressure medications are “safe and effective.”
If you’ve ever tested your blood pressure at home, you’ve probably discovered that you don’t get a consistent number. Numbers can jump up or down 20, 30, or even 40 points in one day — sometimes in just a few minutes. It's different at different times of the day, and no two days are the same. So if your systolic pressure goes from 150 in the doctor’s office to 120 at home, do you have hypertension? Prehypertension? Neither of these? How do you know?
Scientists have long debated the effects of caffeine on blood pressure. But an analysis of 34 studies seems to have delivered a verdict. On average, consuming 200 to 300 mg caffeine (the amount found in one or two cups of coffee) raises systolic blood pressure by 8 mmHg and diastolic blood pressure by 6 mmHg. And the effects can last for up to three hours. (For reference, 8 ounces of drip coffee contain 100-125 mg of caffeine; the same amount of tea, 50 mg; an equal quantity of cola, about 40 mg.)
Blood pressure is written as two numbers, such as 112/78 mm Hg. The top, systolic, number is the pressure when the heart beats. The bottom, diastolic, number is the pressure when the heart rests between beats. Normal blood pressure is below 120/80 mm Hg. If you’re an adult and your systolic pressure is 120 to 139, or your diastolic pressure is 80 to 89 (or both), you have pre-hypertension. High blood pressure is a pressure of 140 systolic or higher and/or 90 diastolic or higher that stays high over time.
3. Eat a healthy diet. Food is another powerful medicine. Whether you need to lose weight or not, eating well can improve your blood pressure. That means eating fruits and vegetables, whole grains, healthy oils (such as olive and canola), foods rich in omega-3 fatty acids (salmon, tuna, walnuts and flaxseed, for example) and two or three servings daily of low-fat or nonfat dairy products. It also means avoiding saturated and trans fats.
Vitamin D. Vitamin D deficiency is associated with increased risk of developing hypertension (20), and large doses of vitamin D (50,000 IU per week) have been shown to lower blood pressure over eight weeks (21). One mechanism by which vitamin D may lower blood pressure is through suppressing renin, which regulates mean arterial blood pressure (22). Sun exposure is an easy and cheap way to get vitamin D.
3. Implement strategies to lower inflammation. Several cross-sectional and longitudinal studies connect high blood pressure with chronic inflammation, a driving force for nearly every disease on the planet. Lowering inflammation starts with what you put on your fork. Focus on anti-inflammatory foods like wild-caught seafood (rich in omega-3 fatty acids), freshly ground flax and chia seeds, spices like turmeric, and plenty of colorful plant foods. Good sleep, stress management, exercise, and the right nutrients can also help lower inflammation.
Consuming 4 tablespoons of flaxseed can lower systolic blood pressure in postmenopausal women who have a history of heart disease, a small study found. The seeds are rich in omega-3 fatty acids, which probably explains the effect. Try 2 tablespoons in your oatmeal or yogurt at breakfast, then sprinkle 2 tablespoons over soup or salad later in the day for a tasty crunch.
